Doctors spend significant time documenting care during and after patient visits. Heavy paperwork demands may also contribute to administrative burden, workflow pressure and burnout. Today, large health systems are investing in ambient documentation technology to help ease that workload and allow physicians to focus more on patient care.

What Is Ambient Clinical Documentation?

Ambient clinical documentation uses AI to listen in on discussions between patients and physicians during encounters. The technology transcribes the conversation, pulls pertinent medical information and compiles it into a draft clinical note for provider evaluation. Some ambient documentation technologies can also immediately populate the approved information in the electronic health record. This technique minimizes the need for physicians to type notes during and after each visit.

Making Documentation Relief a Strategic Priority

Addressing clinician fatigue must include reducing documentation expectations. A 2025 study showed that clinicians employing ambient scribing spent 20.4% less time working on notes per consultation. The technology also reduced their typical after-hours work time by 30%. These changes can give professionals extra time with patients and allow them to perform other obligations during the workday.

Ambient documentation should serve a bigger organizational purpose. Workforce sustainability and the ethical use of AI were the leading topics at the American Nurses Association’s 2025 Membership Assembly. Health systems can integrate these priorities into their strategic planning by defining specific goals to reduce administrative burden and assessing whether the technology enhances physicians’ day-to-day work.

Building Confidence in AI-Supported Care

Healthcare systems may be reluctant to adopt AI in clinical operations because errors could compromise patient safety. However, AI has demonstrated its effectiveness in several crucial areas of healthcare. For example, studies have shown that AI, when combined with radiologists, can achieve 95% accuracy in cancer detection. While diagnostic tools play a distinct role from ambient documentation technology, the effectiveness suggests that well-designed AI may assist clinicians in critical situations.

With ambient systems, clinicians still have to review each note before it is put into the medical record. Health systems can use the technology to minimize the documentation burden with appropriate management and continuous performance monitoring and without removing clinical judgment from the process.

How Health Systems Are Seeing Results

Ambient documentation technology is already showing significant results throughout large health systems. An AI scribe is available to over 4,000 Cleveland Clinic clinicians. The technology decreased time spent drafting and reviewing notes per session by two minutes and 14 minutes per day.

Cooper University Health Care has also adopted ambient documentation within its Epic system. A survey of clinicians across Cooper facilities found that users saved an average of 4.15 minutes per patient, which the organization says can add up to more than an hour a day. Cooper also said they had more detailed notes and better transitions between healthcare teams. The results demonstrate how the technology may enhance patient involvement while driving improved operational efficiencies.

Challenges Health Systems Must Address

Ambient documentation technology presents practical and clinical issues. Health institutions should also consider implementation costs and whether a platform can easily be integrated into their existing EHR. Organizations will also need defined controls for data storage and access because the device collects sensitive patient discussions. Vendors should be HIPAA compliant and should clarify whether recordings are kept.

In a pilot study conducted in 2024, 94.7% of AI-generated clinical notes contained no major errors. This indicates a high degree of accuracy, but the errors that do occur highlight the necessity for clinicians to check each note before signing off. Health systems should regard ambient AI as an aid to clinical judgment, not a substitute for it. Regular checks on accuracy and staff training can help encourage appropriate use.

A Strategic Investment in the Future of Care

Ambient documentation technology enables health organizations to strategically improve administrative efficiency while reducing the documentation burden that fuels clinical fatigue. Organizations employing AI for routine note writing are helping clinicians focus more on the individuals they treat. As technology improves, it may be used to make healthcare administration more efficient while preserving human judgment and patient connection at the heart of treatment.
