Register for the Collaboratives in Action Two-Part Series: 12/5 & 12/12

With heightened awareness of the impacts of social factors on health, screenings, assessments, and referrals to social services and community-based organizations have increased. However, information is not being captured or exchanged in standardized ways, leading to patient distrust, process inefficiencies, and provider burnout. As more social factor data is collected, the use of evidence-based tools and the application of data standards are critical for better patient care and enablement of data aggregation and sharing. The Gravity Project (@thegravityproj) has made significant strides with the development of SDOH standards, but they have not yet been fully implemented across the health and social care ecosystem. Advancing implementation of these standards not only supports individual patient care, but also supports public health and policymaker implementation of programs and policies that advance health equity.

Civitas Networks for Health (@civitas4health), in partnership with the Gravity Project, looks forward to sharing insights and outcomes from their joint program aimed at supporting the implementation of Gravity’s social determinants of health data standards to reduce burden with a focus on public health and health equity use cases.

This public two-part webinar series provides a unique opportunity to access knowledge, best practices, and strategies garnered from this program.

  • Session 1: Supporting Communities through Pilots and Shared Learning – December 5, 3:00-4:00 p.m. ET: This session will highlight how Civitas and Gravity provided targeted training, technical assistance, shared learning opportunities, and relevant resources to support communities with implementation of Gravity social determinants of health data standards. We will highlight successes and opportunities for pilot sites in New York, Oklahoma, Colorado, and Arizona, and discuss how local efforts were highlighted in monthly affinity group meetings to best support the broader health data community. We will also discuss how state public health departments came together through monthly state learning labs to facilitate collaborative learning and knowledge exchange for partners implementing Gravity Project data and exchange standards into state Medicaid waivers.
  • Session 2: Social Care Voice Co-Design – December 12, 3:00 – 4:00 p.m. ET: This session will highlight how Civitas, Gravity, and partner, HealthBegins, brought together over 15 leaders in diverse social care settings to elevate the voice and perspective of providers. share how over a six-session format, we solicited, synthesized, and documented social care provider feedback on Gravity data standards and resources, Gravity-relevant social care roles, requirements, and barriers and opportunities across social care settings. Join us to hear directly from social care leaders about this joint work.

Civitas hosts quarterly Collaboratives in Action events focused on showcasing important industry data-driven, multi-stakeholder collaborations and initiatives. These educational webinars provide opportunities for sharing best practices, lessons learned, and practical insights into solutions. Collaboratives in Action events are open to all Civitas members and the public.
