The Centers for Medicare & Medicaid Services (CMS) are pleased to announce that the American College of Cardiology has endorsed the Million Hearts (@MillionHeartsUS): Cardiovascular Disease Risk Reduction Model (MH Model)! The MH Model test is a randomized-controlled trial designed to identify and test scalable models of care delivery that reduce Cardiovascular Risk. MH Model supports both the Million Hearts’ goal to prevent one million heart attacks and strokes and CMS’ objective to identify and spread better models of care delivery and payment.
This model aims to improve quality while maintaining budget neutrality for Medicare beneficiaries ages 18-79 who have not had a previous heart attack or stroke. CMS will select 720 practices for the demonstration, scheduled to begin in January 2016, and will divide the practices into intervention and control cohorts of equal size.
CMS is encouraged by the enthusiastic response to the MH Model, and encourage you to join this important study. Act now—space is limited! Enroll today to secure your spot in this important study! A complete, detailed description of the Request For Applications (RFA) is now available.
