The ONC Patient Matching Stakeholder Meeting

All participants in this meeting must register in advance. To register for this meeting, please complete and submit the following form with your name, affiliation, organization, e-mail address, etc. Although there is no registration fee to attend the meeting, all attendees must cover their own travel, meals, and incidental costs. Due to space restrictions, we ask that organizations send up to two (2) representatives. We will be providing webinar capabilities, so please indicate below if you wish to attend in-person or remotely. Space is limited, so in-person registrations are available on a space available basis. Once the room is full, registrants will be able to attend via webinar.
